Implementing a First Nations cancer coordinator role: A toolkit for healthcare providers

A toolkit to guide health services in developing and implementing a First Nations care coordinator role. It includes a collection of background material, implementation steps, strategies/activities, recommended outcome measures and resources to support services in their implementation goals.

The toolkit is the culmination of years of work to design a model of cancer care that aligns with the values and needs of First Nations communities. It was developed through a collaboration between WCMICS, St Vincent’s Hospital Melbourne, and the Royal Melbourne Hospital.
