The Cancer Care Coordinator (0.4FTE) plays a pivotal role in the delivery of comprehensive cancer care. This position involves providing clinical leadership, mentoring team members, and delivering essential care and treatment to patients. A core responsibility is the effective coordination of care across various clinicians, disciplines, and healthcare services to ensure a seamless and patient-centred experience throughout the treatment.
Attend Multidisciplinary Team (MDT) meetings and other relevant forums to support the planning and delivery of patient care

Facilitate timely scheduling of appointments, diagnostic tests, and procedures to expedite treatment plans and ensure continuity of care

Support seamless care transitions by establishing clear referral pathways and ensuring timely communication between health professionals and across care settings

Act as a liaison between patients affected by cancer and members of the healthcare team

Undertake comprehensive assessments and screening for clinical, psychological, and supportive care needs at initial presentation and at key points throughout treatment

Provide consistent, timely education and information to patients and their families

Provide clinical leadership and mentorship within the oncology nursing team
